fix(continuous-learning-v2): warn when the observer never survives a hook invocation (#2489) (#2606)
* fix(continuous-learning-v2): warn when the observer never survives a hook invocation (#2489) The observer is lazy-started from a hook process that exits immediately afterwards. start-observer.sh's liveness check runs inside that still-living process tree, so it always sees a healthy observer and prints "Observer started (PID: N)". On native Windows (Git Bash/MSYS2) the reap happens later, when the hook's Job Object closes, so no self-check placed in start-observer.sh can ever observe the failure. The next hook invocation is the only place the death is visible, and _CHECK_OBSERVER_RUNNING already found it there -- then discarded it, deleting the stale PID file and restarting silently, once per tool call, forever. Users were left with an observer-start.log full of success lines and an observer that never completed a single analysis cycle. Record the "well-formed PID that is no longer alive" case, count consecutive non-survivals in ${PROJECT_DIR}/.observer-nosurvive-count, and log one explanatory warning when the streak reaches ECC_OBSERVER_NOSURVIVE_WARN_AFTER (default 3). Warning fires on equality so a persistent failure logs once per streak rather than once per tool call; finding the observer alive resets the streak. The Windows-specific explanation is gated on uname so Linux/macOS users are pointed at observer.log instead of a wrong diagnosis. Counting happens in the caller, not inside _CHECK_OBSERVER_RUNNING, because that function is invoked once per PID file and again under the start lock. The PowerShell backgrounding rewrite is deliberately not included: it cannot be exercised on a non-Windows machine, and untested process-spawning code is a worse outcome than an accurate diagnostic. * docs(continuous-learning-v2): state observer platform support and the new warn threshold The observer's Windows limitation was only discoverable by hitting it. Record it next to observer.enabled, where it is read before the flag is set, and document ECC_OBSERVER_NOSURVIVE_WARN_AFTER so the knob added alongside the warning does not repeat the undocumented-env-var problem tracked in #2573. zh-TW is intentionally left alone: translation parity is not enforced here and the repo rejects blind translation imports without translator review. * fix(continuous-learning-v2): serialize the non-survival streak under the lazy-start lock observe.sh runs on every tool call, so the streak read-modify-write could race between concurrent invocations -- losing an increment or logging the warning twice. That is the same class of bug the signal counter hit in #2296, and this repo's rule is to never fall back to an unlocked read-modify-write. Rather than add a second lock, move the increment into _START_OBSERVER_LOGGED. All three of its call sites already run inside the lazy-start lock (flock / lockfile / mkdir), so the update is serialized with no new machinery. Counting at the restart instead of at detection also means N racing hooks record one death rather than N. The reset stays in the caller: it is an idempotent unlink, not a read-modify-write, so it needs no lock. Adds a regression case pinning the increment inside _START_OBSERVER_LOGGED and asserting all three call sites remain locked. * fix(continuous-learning-v2): harden the non-survival threshold and warning output Three review findings on the #2489 diagnostic: - An all-zero threshold silently disabled it. `00` passes a digits-only check but compares as zero, and the streak only grows, so the warning could never fire. Normalize with base-10 arithmetic and fall back to the default for anything below 1. Base 10 is forced explicitly because a leading zero would otherwise be read as octal, and `08` is an arithmetic error that would abort the hook under `set -e`. The same normalization now guards the streak read. - An unwritable log silently swallowed the diagnostic. Build the message once and fall back to stderr when the append fails. This cannot spam: the block runs once per streak, not once per tool call. The counter write keeps its `|| true` -- observe.sh runs on every tool call and the repo rule is that hooks exit 0 on non-critical errors, so a full disk must not break tool use. - The live-PID test fixture used process.pid, which is 1 in a container and is deliberately rejected by _CHECK_OBSERVER_RUNNING; the reset case would then fail for the wrong reason. Use a spawned child and clean it up. Adds a regression case for the all-zero threshold. Verified on bash 3.2 (the macOS CI runner shell) as well as bash 5. * fix(continuous-learning-v2): warn only on a persisted streak increment If the counter write fails, the file stays below the threshold, so every later hook invocation rereads it, re-increments in memory, hits the equality check and warns again -- turning the once-per-streak diagnostic into once-per-tool- call spam. That is worse in exactly the case the stderr fallback added in the previous commit was meant to cover, since a disk that cannot take the log usually cannot take the counter either. Gate the warning on the write succeeding. The write stays non-fatal: it runs as an `if` condition, so `set -e` is satisfied and an unwritable counter costs a delayed diagnostic rather than a broken tool call. Tests: an unwritable counter must stay silent across repeated invocations while the hook still exits 0, and a leading-zero threshold ("08") must be read as decimal -- "00" alone did not exercise the base-10 conversion, since it is zero either way. --------- Co-authored-by: haelyra <49814733+haelyra@users.noreply.github.com> |

fix(skill-evolution): wire Skill PostToolUse tracker so skill-health shows real runs (#2490)
* fix(skill-evolution): wire Skill PostToolUse tracker so skill-health shows real runs (#2463) recordSkillExecution() had no production callers, so ~/.claude/state/skill-runs.jsonl was never written and `scripts/skills-health.js --dashboard` always reported 0 runs. Adds scripts/hooks/skill-run-tracker.js and registers it as an async PostToolUse hook (matcher: Skill) in posttooluse-dispatcher.js, which is now the single PostToolUse entrypoint on main. Addresses the privacy and bounds review on #2555's sibling PR: - No prompt text is persisted. task_description is synthesized as "Skill invocation: <skill_id>"; tool_input.task_description/description/ prompt are never read. - Every persisted string is bounded and charset-restricted. A skill id is an identifier, so free text, newlines, or an over-long value are dropped rather than truncated and written through. - The JSONL sink is created 0600 and re-tightened on each append, repairing files written before this bound existed. - The sink is capped at MAX_RUN_RECORDS (5000), trimmed oldest-first, so the append-only file can no longer grow without limit. Tests cover the privacy guarantee (no prompt text reaches a record), the identifier bounds, the file mode on POSIX, and the retention cap. * fix(skill-evolution): re-register the tracker for PostToolUseFailure The rebase onto current main dropped the hooks.json entry, which silently resurrected the P1 from the earlier review round: deriveOutcome() still branches on hook_event_name === 'PostToolUseFailure', but the PostToolUse dispatcher does not fan that event out, so the branch was unreachable in production. Hard Skill failures were dropped from telemetry entirely, which inflates the dashboard success rate — the opposite of what #2463 asks for. Restores the dedicated PostToolUseFailure entry (matcher Skill, id post:skill:track, same run-with-flags wrapper and standard, strict gating as the dispatcher registration). Verified end-to-end: a PostToolUseFailure payload piped through run-with-flags now records outcome "failure". Adds a regression test asserting the registration so a future rebase cannot quietly drop it again. --------- Co-authored-by: haelyra <49814733+haelyra@users.noreply.github.com> |

ci: add Python CI job (ruff, mypy, pytest) for the llm-abstraction package (#2364)
* ci: add ruff + mypy to the Python CI job and fix pyproject tool config The python-tests job runs pytest but not lint/type checks, and the ruff and mypy configuration in pyproject.toml was silently broken, so neither tool could run at all. - add ruff and mypy steps to the existing python-tests job - fix invalid pyproject keys: [tool.ruff] src-path -> src, [tool.mypy] src_paths -> mypy_path - ignore ruff UP042 (the (str, Enum) mixin is intentional) - resolve ruff findings (unused/unsorted imports) across src and tests - fix mypy errors in tools/executor.py and prompt/builder.py * fix(ci): satisfy Python lint after main refresh --------- Co-authored-by: haelyra <49814733+haelyra@users.noreply.github.com> |

fix(hooks): context-monitor noise — loop-detection false positives + per-call cost-warning spam (#2486)
* fix(hooks): context-monitor noise — loop-detection false positives and per-call cost-warning spam Two independent noise sources in the PostToolUse context monitor injected agent-facing warnings on nearly every tool call: 1. LOOP WARNING false positives. hashToolCall() hashed only the first 160 chars of a Bash command, so distinct long commands sharing a prefix (heredocs, long one-liners) collided and consecutive DIFFERENT calls looked like a stuck loop. Additionally LOOP_THRESHOLD=3 against a 5-entry ring buffer fired on legitimate repetition (retries, polling). Fix: hash the full command (digest truncated, not the input — same treatment the Edit/Write branch already got), and require all 5 of the last 5 calls to be identical before warning. 2. COST NOTICE spam. run() deduped warnings on exact message text, but the cost figure embedded in the text moves on nearly every call, so once a session crossed $5 a 'new' COST NOTICE was injected per tool call for the rest of the session. Context warnings had the same defect via the remaining-% figure. Fix: dedupe on a stable per-tier key (cost:notice/warning/critical, context:warning/critical, scope) so each tier fires exactly once and re-fires only on genuine escalation. The existing ECC_CONTEXT_MONITOR_COST_WARNINGS opt-out is unchanged. Tests: loop threshold updated (5-of-5 fires, 4-of-5 does not), long shared-prefix Bash hash regression, and a run()-level tier-dedupe test (notice fires once, silent on cost tick, re-emits on escalation). feat(session-start): rank injected instincts by project/stack relevance (#2466)
* feat(session-start): rank injected instincts by project/stack relevance Instinct selection at SessionStart ranked purely by confidence, so a high-confidence instinct about an unrelated stack could take an injection slot from a lower-confidence instinct that is actually relevant to the current project. Rank by confidence + location/stack relevance instead: project-scoped instincts, and instincts whose domain/trigger matches the detected stack (languages/frameworks via detectProjectType, plus terraform/dbt markers), get a small additive boost. The confidence>=threshold floor and the injection cap are unchanged, and ranking degrades to confidence-only when nothing matches or when ECC_INSTINCT_RELEVANCE_RANKING is set to off. The ranking helpers live in scripts/lib/instinct-relevance.js with unit coverage in tests/lib/, plus an end-to-end ordering test in tests/hooks/. Completes part (b) of #2371; part (a) (configurable count + threshold) shipped in #2413. Fixes #2371 * refactor(session-start): drop redundant confidence tiebreaker in instinct sort Greptile flagged that the secondary `right.confidence` comparison in summarizeActiveInstincts' sort was dead code when relevance ranking is disabled and, when enabled, was reached only on a floating-point tie of the combined score — where it skipped the intended scope-label tiebreaker. Remove it: the primary combined-score comparison already reduces to confidence-only ordering when relevance is off, so behavior there is unchanged; a genuine combined-score tie now falls through to the documented scope-first, then id, order. * test: isolate instinct relevance environment --------- Co-authored-by: haelyra <49814733+haelyra@users.noreply.github.com> |

fix(plan-canvas): deliver browser chat to the agent every time (#2739)
Feedback sent from the canvas only reached an agent through a live /api/await long poll. When a turn ended with no await parked, queueFeedback wrote the message to sessions.json and nothing ever consumed it, so sending appeared to do nothing at all. The presence pill made it worse: workingKeys had no expiry and the feedback handler never broadcast presence, so it froze on "agent working" while nobody was listening. Delivery: - Add the stop:plan-canvas-pending hook. It drains undelivered feedback and blocks the Stop, handing the messages to the agent, so a canvas message lands even when no await is running. Scoped to sessions under cwd so parallel agents cannot swallow each other's feedback; set ECC_PLAN_CANVAS_STOP_SCOPE=all to widen. Honors stop_hook_active and fails open on every error path. - run-with-flags.js did not await a hook's run(), so any async hook silently degraded to pass-through. Fixed; plan-canvas-pending is the only async hook today. Presence and indicators: - Presence is now ended/typing/thinking/listening/queued/waiting. thinking and typing self-expire (90s/30s) and a 5s sweep pushes the decay to an idle browser, so the pill can no longer stick. - Broadcast presence when feedback is queued, and clear the activity state when an agent reply lands. - Add POST /api/session/:key/typing so agents can drive the indicator. - Chat shows an animated dots bubble for thinking and typing, plus an explicit note when a message is queued with nobody listening. Respects prefers-reduced-motion. - Send status reports what actually happened instead of always claiming the agent will pick it up. CLI and skill: - Add `ecc-plan-canvas pending` and `typing <file> --state ...`. - SKILL.md documents background await as the primary pattern and makes replying in the canvas mandatory. Tests: 6 new server cases covering queued presence, the typing endpoint, state expiry and the sweep, plus a new hook suite covering delivery, drain-once, stop_hook_active, cwd scoping and fail-open. fix(hooks): never format installed plugin and marketplace clones (#2667)
The Stop hook formats every JS/TS file edited during a response, grouped by the project root each file happens to sit in. That includes trees under .claude/plugins, which are third-party checkouts we only read. Formatting them writes to code the user does not own. It also does real damage when a repo's committed code has drifted from its own formatter config: the rewrite is not a no-op but a wholesale reformat, so an unrelated bugfix ends up carrying hundreds of untouched lines. I hit this contributing to this repo — a 162-line fix arrived as a 478-line diff, most of it reformatted code the change never went near. Skips both the user-level install root and a project-local one, mirroring the lookup in scripts/harness-audit.js. Paths are resolved before the prefix comparison, and a sibling such as .claude/plugins-backup does not match. The user own .claude config outside plugins is still formatted. Adds 7 tests for the predicate, plus an end-to-end check that a clone file listed in the accumulator is left byte-identical. Suite 16 to 23. fix(hooks): catch the bypass short flag anywhere in a cluster (#2668)
isCommitNoVerifyShortFlag anchored on the first character, so it only recognised the flag when it led the cluster. Git clusters short options, which means git commit -an is -a plus the bypass flag and skips the hooks. -sn and -vn slip through the same way, while -na and -nm are caught — the difference is position, not intent. Scanning now walks the cluster and stops at a value-taking option, since that option swallows the rest as its inline value. The n in -mn stays message text, and the existing -tn case keeps working. Adds 4 tests: the three clustered forms that were escaping, plus -mn to pin the inline-value boundary. Verified the three fail against current main. Suite 25 to 29. fix(hooks, lib): fix hook detection and parsing edge cases (#2405)
* fix(hooks, lib): fix hook detection and parsing edge cases
- auto-tmux-dev: dev\b -> dev(?![\w-]) so one-shot dev-build/dev-docs scripts
are not detached into tmux; align command shapes (yarn run dev, bun dev) with
pre-bash-dev-server-block.js DEV_PATTERN.
- pre-bash-commit-quality: skip obvious non-secret placeholders (env refs,
${...}, <...>, whitelisted tokens) in the api-key rule without suppressing
real high-entropy secrets; make -m message extraction quote- and
escaped-quote-aware so `-m "fix: \"x\""` / apostrophes are not truncated.
- pre-compact: annotate the CURRENT worktree's session (match **Worktree:** /
legacy **Project:**) instead of the newest *-session.tmp across all projects,
layered onto the LLM-summary flow from #2388; a present-but-blank Worktree
header is treated as non-legacy (no foreign project fallback).
- shell-substitution: stop double-appending a trailing backslash in an
unterminated backtick span.
- utils readStdinJson: on overflow, settle and resolve {} immediately (clear
timer + listeners) instead of waiting for end/timeout and parsing a partial
prefix; surface the overflow on stderr.
Regression tests added/extended (new tests/hooks/pre-compact.test.js).
Addresses review feedback on #2405. The earlier block-no-verify change was
dropped: its message-value skip on merge/cherry-pick/am/rebase would let
`git rebase -m --no-verify` bypass the hook (rebase's -m is the boolean
--merge), a false-negative worse than the contrived false-positive it fixed.

fix(hooks): dedupe transcript usage by message.id in cost-tracker (~2.5-3x inflation) (#2483)
Claude Code writes one transcript JSONL line per content block, so a single API response (one message.id) spans multiple assistant lines that each repeat the same message.usage. sumUsageFromTranscript summed every line, inflating token totals and estimated_cost_usd roughly 2.5-3x. Verified on a real session: 704 assistant lines but only 286 unique message.ids (2.46 lines/response on average); line-summing reported $866.52 while the deduped total is $332.62. Usage payloads are identical across lines of the same id (0/286 varied), so counting once per id is equivalent to taking the last line per id. Fix: collect usage into a Map keyed by message.id (last line wins) and sum unique entries. Lines without a message.id (older transcript shapes) keep the previous per-line behavior via a synthetic key, so existing tests and old transcripts are unaffected. Adds a regression test: a response split into 3 content-block lines with the same message.id is counted exactly once. Note: rows already written to ~/.claude/metrics/costs.jsonl by the old code carry inflated token counts and estimates (except rows whose cost came from the harness-cost cache, where cost is authoritative but token counts are still inflated). Downstream consumers may want to annotate history; this change intentionally does not rewrite the raw log. Co-authored-by: Claude Fable 5 <noreply@anthropic.com> |

fix: detect Anthropic API keys (sk-ant-...) in pre-commit secret scan (#2529)
The existing OpenAI pattern sk-[a-zA-Z0-9]{20,} never matches real
Anthropic keys: their sk-ant-api03-... format contains hyphens, which
break the character class before reaching the 20-char threshold. Keys
from the fastest-growing Claude Code user base slipped through the scan.
Adds a dedicated sk-ant-[a-zA-Z0-9_-]{20,} pattern (checked before the
OpenAI one) and extends the staged-secrets test with a realistic
Anthropic key fixture.

fix(observer): repair daemon boot (stale resolver name) and close analysis stdin (#2452) (#2464)
Two continuous-learning-v2 observer regressions reported in #2452: - start-observer.sh still called _ecc_resolve_homunculus_dir, but the shared lib was renamed to _clv2_resolve_homunculus_dir (with observe.sh and detect-project.sh updated, start-observer.sh missed). Under set -e every launch dies with exit 127 at line 40 - daemon boot is broken on all platforms, not just Windows. - observer-loop.sh backgrounds the analysis claude call with stdin left open; on Git Bash/MSYS2 the child inherits it, waits, warns 'no stdin data received', and exits 1 before reading the analysis file. Close stdin with </dev/null while keeping the -p prompt flag, preserving the Windows-compat decision from #842 instead of reverting to a stdin redirect. Adds two source invariant guards to tests/hooks/hooks.test.js: every *_resolve_homunculus_dir call site must match a function the shared lib defines, and the backgrounded claude call must close stdin. fix(config-protection): match protected filenames case-insensitively (#2543)
On a case-insensitive filesystem (macOS APFS/HFS+, Windows NTFS) a write to `.ESLINTRC.JS` lands on the exact same inode as `.eslintrc.js`, but the guard looked the basename up in PROTECTED_FILES with a case-sensitive `Set.has`. Every entry in that Set is lowercase, so any case-variant path missed the branch entirely and returned exit 0 — a single Write silently overwrote a live config while the hook reported success. Reproduced on macOS APFS: `.eslintrc.js` and `.ESLINTRC.JS` share one inode, yet the hook returned exit 2 for the former and exit 0 for the latter, and the uppercase write replaced the real config's contents. This is a one-step bypass of the whole guard and needs no shell access, unlike the known delete-then-recreate route. Fix: also test `basename.toLowerCase()`. All 32 PROTECTED_FILES entries are already lowercase, so the fallback is exact. On a genuinely case-sensitive filesystem this costs at most a false positive on a distinct file whose name differs from a protected one by case alone. Behaviour deliberately unchanged: first-time creation is still allowed (the bootstrap affordance), non-config paths still pass through, and the existing lstat/ENOENT fail-closed semantics are untouched. Test: adds a case-variant case that asserts exit 2. It guards itself with an inode comparison and skips on case-sensitive filesystems rather than asserting something untrue there. Verified in both directions — it FAILS against the unpatched hook (`Got 0; 0 !== 2`) and passes with the fix. Suite: 9/9. |

fix(hooks): remove stray '?' that made every 'yarn <anything>' fire tmux reminder (#2517)
* fix(hooks): remove stray '?' that made every 'yarn <anything>' trigger tmux reminder
The tmux-reminder matcher uses one alternation per package manager. Each
branch requires a subcommand (install|test) — except yarn, whose subcommand
group carried a trailing `?`:
yarn (install|test)?
That made the subcommand optional, so the branch degraded to "yarn " plus
anything: `yarn add foo`, `yarn build`, `yarn dev`, even `yarn --version`
all matched and spammed the "Consider running in tmux" hint into the
additional-context channel.
Drop the `?` so yarn matches parity with npm/pnpm/bun. Verified locally
against 14 cases (yarn install/test still fire; yarn add/build/dev/… no
longer do; npm/pnpm/bun/pytest behavior unchanged).
Fixes #2514
* test(hooks): add pre-bash-tmux-reminder regression tests
Add coverage for the tmux-reminder matcher following the auto-tmux-dev.test.js
structure — the regex-first hook now has direct regression tests for the yarn
branch fix in this PR (and for the sibling package managers, other matched
tools, TMUX bypass, and malformed input).
16 assertions total:
- fires for: yarn install, yarn test, npm install, pnpm test, bun install,
pytest tests/, cargo build
- does NOT fire for: yarn add react, yarn build, yarn dev, yarn --version,
bare `yarn`, npm run dev
- respects TMUX env var
- tolerates invalid JSON and missing command field
Verified the tests actually catch the bug: reintroducing the buggy
`yarn (install|test)?` fails 4 of the 5 yarn non-match cases (the fifth,
bare `yarn`, stays passing because even the buggy branch requires a trailing
space after yarn).
Addresses CodeRabbit review on #2517.
* test(hooks): fail loudly on spawn errors, use destructuring, split runTests
Address three CodeRabbit review notes on tests/hooks/pre-bash-tmux-reminder.test.js:
- Fail loudly on spawnSync errors: raise instead of coercing
`result.status || 0`, which would mask spawn errors, timeouts, or signal
termination as a successful exit 0 (masks legitimate test failures).
- Use destructuring (`const { TMUX, ...env } = process.env`) instead of
copy-then-`delete` so the base env is built immutably.
- Split `runTests` (was 66 lines) into small per-group helpers
(runYarnTests, runSiblingPackageManagerTests, runOtherToolTests,
runTmuxBypassTests, runEdgeCaseTests). `runTests` is now 18 lines and
purely orchestrates.
16 assertions still pass; no coverage changes.
The 4th CodeRabbit note (avoid console.log in test files) is intentionally
not adopted here — every sibling hook test in this repo
(auto-tmux-dev.test.js, bash-hook-dispatcher.test.js, block-no-verify.test.js,
etc.) writes to console.log because the project's own test runner
(tests/run-all.js) is console-log based and there is no Jest/Mocha
dependency. Diverging from the established convention in a bugfix PR is
out of scope.

refactor(hooks): consolidate PostToolUse hooks into sync/async dispatchers (#2494)
* refactor(hooks): consolidate PostToolUse hooks into sync/async dispatchers Replace 10 individual PostToolUse entries in hooks.json with two consolidated dispatcher entries (post:dispatcher:sync / post:dispatcher:async). The dispatcher's internal registry preserves every hook ID, matcher, and profile, so ECC_DISABLED_HOOKS and ECC_HOOK_PROFILE gating behave exactly as before. Performance (Edit event, actual hooks.json commands spawned in parallel like the harness does, median of 7 runs): - Blocking hook latency: 81ms -> 49ms (~40% faster; 7 blocking processes -> 1 sync dispatcher) - Node processes per tool call: 10 -> 2 (7 blocking + 3 async -> 1 sync + 1 async) - observe-runner now runs in-process (~370ms) inside the async dispatcher, which stays backgrounded (async: true, timeout 45s), so it adds no user-facing latency. Also: - dashboard-web lists dispatcher-managed child hooks so the hook inventory stays complete - post-edit-console-warn refactored to export run() for in-process dispatch while keeping standalone stdin behavior - dispatcher stdin reading is multi-byte safe (StringDecoder) and child hook exit codes propagate to the dispatcher exit code * test(hooks): replace emoji literal with unicode escape for CI unicode safety check * fix(hooks): adopt explicit cli() entrypoint and merge multi-hook stdout Address Greptile review on #2494: - Replace the non-standard 'require.main === undefined' guard with an explicit exported cli(). The hooks.json bootstraps now call require(s).cli(), so merely requiring the module (dashboard-web, test runners, Jest, worker threads) can never trigger dispatch, attach stdin listeners, or set process.exitCode. - Replace last-writer-wins stdout with mergeHookStdout(): when several hooks emit additionalContext envelopes they merge into a single PostToolUse envelope; non-mergeable raw stdout keeps the last hook's output and emits a stderr warning naming the dropped hook IDs, so nothing is lost silently. Also includes local formatter reformatting of the dispatcher and its test file (no behavioral changes beyond the above). * fix(hooks): keep post:bash:dispatcher phase reachable in minimal profile The Greptile P1 premise was partially incorrect: sub-hooks without explicit profiles default to standard, strict via parseProfiles() (scripts/lib/hook-flags.js), so audit/cost logs never ran under the minimal profile on main either — there is no user-visible regression. However, main did spawn the bash dispatcher phase unconditionally and let each sub-hook gate itself. Restore that semantic by opening the outer registry gate to minimal, standard, strict so a future sub-hook that opts into minimal is not silently blocked at the phase level. Adds the previously missing minimal-profile async dry-run test. * test(hooks): assert failing hook exit code propagates to real process status Spawns the actual dispatcher subprocess with an injected failing hook and asserts the OS-level exit status, stderr diagnostic, and suppressed pass-through — closing the E2E gap CodeRabbit flagged on #2494. * chore: retrigger CI (flaky windows powershell bootstrap test) |

feat(gateguard): add GATEGUARD_EXEMPT_GLOBS path exemptions (#2432)
The fact-forcing gate fires once per first-touched file per session. In
build-heavy sessions this costs a deny->retry round-trip on every new file,
including trees where the gate's questions ("who imports this? what schema?")
carry no signal: test files, generated artifacts, scratch dirs.
Add an opt-in, comma-separated glob allowlist read from GATEGUARD_EXEMPT_GLOBS.
A matching Edit/Write/MultiEdit target skips the first-touch gate; destructive-
Bash and routine-Bash gates are untouched. Default-off (unset => identical prior
behavior), fail-open (a malformed glob is dropped, never throws), and memoized on
the env value, matching the existing getExtraDestructiveRegex idiom.
"env": { "GATEGUARD_EXEMPT_GLOBS": "**/tests/**,**/scratchpad/**" }
Adds 4 tests; all 144 gateguard tests pass.

chore(continuous-learning-v2): standardize shell shebangs to env bash (#2401)
* chore(continuous-learning-v2): standardize shell shebangs to env bash Three scripts under skills/continuous-learning-v2/ used the hardcoded `#!/bin/bash` shebang while the other four already used the portable `#!/usr/bin/env bash`: - hooks/observe.sh (runs on every hook invocation) - scripts/detect-project.sh - agents/start-observer.sh The hardcoded interpreter path fails to execute on systems where bash is not installed at /bin/bash (NixOS, some Homebrew layouts, FreeBSD). Standardize all three to `#!/usr/bin/env bash`, matching the repo-wide majority convention, and add a regression test that asserts shebang uniformity for every shell script in this skill so the inconsistency cannot reappear. Fixes #2303 * test(continuous-learning-v2): harden shebang test runner Address review feedback on the shebang-consistency regression test: - firstLine() now splits on /\r?\n/ so a script checked out with CRLF line endings does not leave a trailing carriage return that would break the shebang comparison on Windows. - The test() helper now surfaces the full error (stack trace, not just the message) on failure and writes pass/fail lines via process.stdout/stderr so diagnostics are preserved. * test(continuous-learning-v2): skip hidden dirs in shebang scan The recursive shell-script scan now skips hidden directories (e.g. the observer's runtime `.observer-tmp`). This keeps the shebang-consistency check deterministic: only committed skill scripts are examined, and an untracked local artifact left over from an observer run can no longer cause a false failure. |

feat(session-start): make instinct injection count and confidence threshold configurable (#2413)
* feat(session-start): make instinct injection count and confidence threshold configurable Expose ECC_MAX_INJECTED_INSTINCTS and ECC_INSTINCT_CONFIDENCE_THRESHOLD so operators can tune SessionStart instinct injection without editing source. Defaults are unchanged (6 instincts, 0.7 confidence floor). The two previously hardcoded constants become DEFAULT_-prefixed fallbacks, resolved through getMaxInjectedInstincts() and getInstinctConfidenceThreshold(), mirroring the existing getSessionRetentionDays() / getSessionStartMaxContextChars() env-override pattern already in this file. Invalid or out-of-range values fall back to the defaults. Adds subprocess coverage in tests/hooks/hooks.test.js and documents both variables in the README Hook Runtime Controls section. Implements part (a) of #2371. * fix(session-start): reject partial env values for instinct injection knobs Parse ECC_MAX_INJECTED_INSTINCTS and ECC_INSTINCT_CONFIDENCE_THRESHOLD with Number() (after trim) instead of parseInt/parseFloat, so malformed values like "3.9", "6abc", or "0.7x" fall back to the default rather than silently accepting the numeric prefix (parseInt("3.9")=3, parseFloat("0.7x")=0.7). Adds a regression assertion that a non-integer count falls back to 6. * fix(session-start): validate decimal grammar for instinct injection env vars Number() still accepts non-decimal numeric syntax, so ECC_INSTINCT_CONFIDENCE_THRESHOLD=0x1 resolved to 1 and ECC_MAX_INJECTED_INSTINCTS=1e2 to 100. Gate each value on a strict format (/^\d+(\.\d+)?$/ for the 0-1 threshold, /^\d+$/ for the positive-integer count) before converting, so hex/exponent/partial values fall back to the default. Adds regression assertions for 1e2 and 0x1. |

fix(observer): replace hardcoded sleep 2 with PID file poll in start-observer.sh (#2356)
* fix(observer): replace hardcoded sleep 2 with PID file poll in start-observer.sh Fixes #2295 The previous `sleep 2` after launching the observer loop has two problems: on slow filesystems or loaded systems 2 seconds may not be enough, producing a false-negative on the subsequent PID file check; on healthy systems it adds unnecessary latency. Replace with a poll loop that exits as soon as the PID file appears: for _i in $(seq 1 50); do [ -f "$PID_FILE" ] && break; sleep 0.2; done 50 × 0.2s = 10s max wait (vs the previous fixed 2s), but typical startup returns within the first iteration. No behavior change in the success path — only the wait strategy changes. Tests: `node tests/run-all.js` 2891 passed / 0 failed; `npm run lint`, `catalog:check`, `command-registry:check` all clean. * test(observer): add regression guard for sleep-2 → PID-file poll (#2295) Asserts start-observer.sh never reverts to the fixed `sleep 2` wait and keeps the 50 × 0.2s `$PID_FILE` poll in place. Sits next to the existing observer-loop invariant block in tests/hooks/hooks.test.js, matching the repo's pattern of guarding shell-script invariants via source-content assertions. Without this, any future "cleanup" that reintroduces a fixed sleep would silently regress the slow-filesystem fix from the previous commit. * fix(observer): loosen poll-regression assertions and document failure-path latency (#2356 review) Addresses CodeRabbit + Greptile feedback on PR #2356: - tests/hooks/hooks.test.js: split the over-specific positive assertion (which pinned the exact `for _i in $(seq 1 50); … sleep 0.2; done` line) into three intent-based assertions — bounded iteration count, early-exit on $PID_FILE, sub-second interval. Valid refactors (rename loop var, switch to `while`, retune to 100 × 0.1s) no longer false-fail while the `sleep 2` regression remains guarded. - start-observer.sh: extend the inline comment to record the trade-off Greptile flagged — a loop that crashes before writing $PID_FILE is now detected in ~10s instead of ~2s. Healthy startups still return in iteration 1. Tests: node tests/run-all.js (Node v22.18.0) → 2892 passed / 0 failed. |

Each copy inlined the full ~700-char plugin-root search using a spread over nested array literals (p.join(d,'plugins',...s) over [['ecc'],...]), which breaks Windows hook execution due to shell quoting (#2368). Collapse every copy to a 250-char locator that loads the committed resolve-ecc-root module and delegates to resolveEccRoot() — no spread, no nested array literals, no escaped double quotes. The real search logic now lives in one tested module. Also route session-start-bootstrap.js through resolveEccRoot() instead of its own duplicated reimplementation, and fix the auto-update.md 'marketplace' (singular) typo along the way. Guard tests updated: discovery behavior is asserted against resolveEccRoot(); the inline is asserted to delegate and to contain no Windows-fragile constructs. Co-Authored-By: Devin AI <158243242+devin-ai-integration[bot]@users.noreply.github.com> * fix(resolve-ecc-root): restore full env-unset discovery in inline resolver Address Greptile review on #2410: when CLAUDE_PLUGIN_ROOT is unset the delegating inline could only load the resolver module from ~/.claude, returning ~/.claude without ever reaching the plugin/cache search. Restore the old inline's discovery breadth (exact plugin roots + versioned cache) Windows-safely (no spread, nested arrays, or escaped quotes), then delegate the authoritative decision to resolveEccRoot(). feat(continuous-learning-v2): make observer model configurable via ECC_OBSERVER_MODEL (#2390)
* feat(continuous-learning-v2): make observer model configurable via ECC_OBSERVER_MODEL
The observer hardcoded `--model haiku`. Parameterize as "${ECC_OBSERVER_MODEL:-haiku}": the haiku default is preserved (no behavior change for existing users), but users can opt into a stronger model — e.g. `ECC_OBSERVER_MODEL=opus` — for higher-quality instinct extraction. Useful on subscription plans where model cost isn't the limiting factor.
* fix(continuous-learning-v2): address review — update wiring test + docs
- Update source-inspection test to assert the ${ECC_OBSERVER_MODEL:-haiku} defaulting behavior (was matching the literal `claude --model haiku`, which this PR changed). All 31 tests pass.
- Add guidance to raise ECC_OBSERVER_TIMEOUT_SECONDS for slower models (e.g. opus) so the 120s watchdog doesn't kill analysis mid-run.
- Fix now-stale 'Haiku session' comment -> 'observer session' (model is configurable).

fix(clv2): archive observations only after successful analysis in observer-loop (#2386)
analyze_observations moved observations.jsonl into observations.archive/
unconditionally, even when the Claude analysis failed (timeout, non-zero
exit, rate limit). Because the analyzer only reads the live file, a failed
batch was archived and never re-analyzed, silently dropping the instincts
it would have produced.
Return early on a non-zero analysis exit so the archive mv runs only on
success, retaining observations for the next cycle to retry. Resolve the
script's own directory from ${BASH_SOURCE[0]} (SCRIPT_DIR) so sibling
scripts (session-guardian.sh) and relative helpers resolve correctly under
both execution and sourcing, and add a source-guard so observer-loop.sh can
be sourced without starting the loop. Add a regression test covering both
the failure (retain) and success (archive) paths.

fix(clv2): surface SIGALRM timeout drops in observe.sh (#2373)
* fix(clv2): surface SIGALRM timeout drops in observe.sh The inline-Python observation writers in observe.sh arm a signal.SIGALRM alarm (8s) so they self-terminate before the async hook's 10s timeout can orphan them (#2278). The handler _ecc_bail called sys.exit(0) with no logging, so when the alarm fired the in-flight observation was silently dropped: nothing was logged, no partial write occurred, and the shell saw a clean exit. There was no way to detect or count how many observations were being lost. Add a single stderr visibility line to both _ecc_bail handlers (the parse-error fallback path and the main observation-writing path) before sys.exit(0), using the repo's "[observe]" log prefix. Exit code stays 0: in a Claude Code hook a non-zero exit signals a block, so changing it would turn an internal timeout into a user-facing tool block. The warning goes to stderr (not stdout) because both blocks redirect stdout into the observations file. Add tests/hooks/observe-signal-timeout.test.js: a static regression guard that every _ecc_bail handler logs to stderr before exiting and keeps exit 0, plus a behavioral check that runs the real handler text extracted from observe.sh and confirms a fired alarm exits 0 and emits the [observe] warning on stderr only. Fixes #2300 * test(clv2): exercise both _ecc_bail handlers end-to-end The behavioral SIGALRM-fire test ran only handlers[0] (the parse-error fallback path); the main observation-write path (handlers[1]) was covered only by the static regex guard. The write path is the higher-value one to verify end-to-end since it carries valid, parseable data that would succeed given more time, so a silent drop there is the worst case. Loop the behavioral check over every extracted handler so a regression that silenced the second handler's stderr write is caught at runtime, not just by the static guard. * test(clv2): select timeout handlers by marker, not array index The behavioral check looped over all extracted _ecc_bail handlers by index. If an unrelated _ecc_bail were ever added to observe.sh, the loop would either test the wrong block or be diluted. Filter the handlers to those carrying the "[observe] SIGALRM timeout" marker so the live SIGALRM check stays pinned to the two #2300 timeout handlers regardless of array order or future additions. * test(clv2): fail fast when python is missing in SIGALRM check The behavioral test returned early when no python interpreter was found, which the test harness records as a PASS — so the SIGALRM contract could go entirely unverified yet still look green. Throw instead, matching the existing insaits-security-monitor convention of failing when a required Python runtime is absent, and drop the in-test console.log. |

fix(clv2): serialize observer signal-counter to stop dropped increments (#2372)
observe.sh bumps the SIGUSR1 throttle counter in
${PROJECT_DIR}/.observer-signal-counter with an unlocked read-modify-write.
The hook runs on every tool call, so concurrent invocations read the same
value, both increment, and lose a write, signaling the observer at
unpredictable intervals and defeating the #521 throttle.
Serialize the read-modify-write under a lock, and only ever bump the counter
while that lock is held:
- Prefer flock with a bounded -w wait (the OS auto-releases it when the fd
closes or the process dies, so there is no stale lock and no lost increment);
on a timeout the tick is skipped rather than bumped unlocked.
- Fall back to an atomic mkdir lock on platforms without flock, with a bounded
spin. An EXIT trap cleans up on normal completion; INT/TERM traps release the
lock and exit, so a signal cannot drop the lock and then continue the
read-modify-write without ownership. If the lock cannot be acquired in the
budget the tick is skipped rather than raced. No hand-rolled PID stale-reclaim
(which is racy and can delete a live re-acquirer's lock).
- Guard the counter read against a corrupt (non-integer) file that would abort
the hook under set -e.
Add tests/hooks/observe-signal-counter-race.test.js: 20 concurrent observe.sh
invocations must not lose increments (exact under flock; at most one dropped on
the best-effort mkdir fallback), the runner rejects on any hook execution
failure or hang, plus content guards for the lock and the corrupt-counter
handling.

fix(windows): prefer PowerShell over bash to prevent zombie process accumulation (#2346)
* fix(windows): prefer PowerShell over bash to prevent zombie process accumulation On Windows, ECC hook scripts were spawning bash.exe (MSYS2/Git Bash) on every tool use via findShellBinary(). Changes to scripts/hooks/plugin-hook-bootstrap.js: - Add isPowerShellBin(bin) helper: basename-based detection so full paths like C:\Windows\...\powershell.exe are handled correctly - findShellBinary(): check BASH env var first (preserves escape hatch), then on win32 probe pwsh.exe -> powershell.exe -> bash.exe -> bash; use correct probe args per shell type; cache result in _cachedShell - findBashBinary(): separate cached bash-only finder used by spawnShell .sh fallback; skips PowerShell binaries even if BASH points to one - spawnShell(): use isPowerShellBin() to select -NoProfile -NonInteractive -File args for PowerShell; .sh scripts fall back to findBashBinary() with a skip-warning if no bash found on Windows observe-runner.js is intentionally unchanged: it always invokes observe.sh which is bash-only; routing it through PowerShell would silently break it. fix(hooks): quote args when probing Windows .cmd MCP servers via shell (#2343)
On Windows, when a bare-name MCP server command (e.g. codesys-mcp-sp21-plus) falls back to the .cmd candidate, the probe sets shell:true to work around Node 18.20+ CVE-2024-27980. However, passing an args array alongside shell:true causes Node to concatenate the tokens without quoting (DEP0190), so an arg containing a space (e.g. --codesys-path "C:\Program Files\...") is re-split by cmd.exe at every space boundary. The child process receives a truncated path, fails to launch, and the probe declares the server unavailable, falsely blocking every MCP tool call to that server. Fix: add a quoteWin() helper that double-quotes any token containing whitespace or cmd metacharacters. In the useShell branch, build a single properly-quoted command line string and pass it as the sole argument to spawn() with no separate args array. The else branch (shell:false, all non-.cmd commands) is unchanged. Regression test added: on Windows, creates a .cmd shim that echoes its first positional argument to stderr, probes it with a space-containing path arg, and asserts the probe succeeds and the arg was not split at the space boundary. Paths containing regex metacharacters (e.g. /home/user.name, /home/c++dev, /home/user (work)) made the match over-broad or invalid, causing either a false negative (live observer missed, migration proceeds and risks registry corruption) or a false positive (migration blocked unnecessarily). Escape the ERE metacharacters in $HOME via sed before building the pattern so the home prefix is matched literally while the trailing .*observer-loop\.sh regex is preserved. Portable across BSD and GNU sed. Fixes #2301 * test(clv2): add regression test for migrate-homunculus.sh $HOME escaping Guards the #2301 fix: extracts the script's sed escaping command and asserts the resulting pgrep -f pattern matches the literal home path while no longer over-matching a regex-expanded decoy (HOME=/home/user.name must not match /home/userXname). Also pins that the guard uses escaped_home rather than $HOME directly. Follows the existing clv2 shell-test convention in tests/hooks/observe-entrypoint-allowlist.test.js. Refs #2301 * test(clv2): skip migrate-homunculus escaping test on Windows The test relies on POSIX bash/sed/grep -E semantics, which differ on the Windows CI runners. Guard with the same process.platform === 'win32' early exit used by tests/hooks/observe-subdirectory-detection.test.js so the bash-dependent assertions only run on POSIX platforms. Refs #2301 |

fix(security): gateguard classifier bypasses (GHSA-4v57) + Windows CI + claw ReDoS
- gateguard (GHSA-4v57-ph3x-gf55): add a quote-aware detection pass that
dequotes command words and splits on UNQUOTED separators incl. newlines, so
newline-separated commands, quoted command words ('rm'/"rm"), quoted
find -exec, and sh/bash -c wrappers are all classified destructive. Additive —
existing 133 cases still pass; +7 bypass regressions + a false-positive guard
(rm inside a quoted echo arg stays allowed). 140/140.
- Windows CI: format-code.ts emitted backslash paths via path.normalize, breaking
forward-slash assertions on all Windows matrix cells — force forward slashes.
- claw.js (CodeQL #1 js/polynomial-redos): bound parseTurns input so the lazy
[\s\S]*? body can't drive O(n^2) scanning on adversarial history files.
Full suite 2852/2852; lint green.

fix(gateguard): check isDestructiveFindExec on each command segment to close compound-command bypass (#2292)
* fix(gateguard): check isDestructiveFindExec on each command segment
`isDestructiveBash` called `isDestructiveFindExec` only on the raw full
command string. When the raw string starts with a non-find command (e.g.
`echo x && find . -exec rm {} \;`), `isDestructiveFindExec` checks
tokens[0] and returns false — then the per-segment loop never calls it
again, letting the destructive `find -exec rm` segment through silently.
Fix: call `isDestructiveFindExec(segment)` inside the per-segment loop so
compound commands (`&&`, `;`, `|`) cannot be used to prepend a harmless
command and bypass the find-exec destructive check.
Adds three regression tests covering `&&`, `;`, and `|` bypass patterns.
* fix(gateguard): use raw body segments for isDestructiveFindExec to close quoted-binary gap
The previous per-segment call passed quote-stripped output from
splitCommandSegments to isDestructiveFindExec, so a quoted exec binary
like find . -exec 'rm' {} \; would arrive as find . -exec {} \; and
the check would silently miss it.
Switch to splitting collectExecutableBodies output on [;|&]+ without
quote-stripping first, so the find-exec binary name is always intact
when isDestructiveFindExec inspects it. This also covers || and
background & separators that the original tests did not exercise.
Adds a regression test for the || OR-chain bypass pattern.
Addresses Greptile review comments on PR #2292.

fix(hooks): stop pre/post Bash dispatcher from echoing the input event (#2240)
runHooks() returned the unmodified raw stdin (the PreToolUse/PostToolUse
input event) on stdout whenever no sub-hook produced additionalContext.
Claude Code parses a hook's stdout as JSON and validates it against the
hook-output schema, so echoing the input object
({session_id, hook_event_name, tool_name, tool_input, ...}) fails with
"Hook JSON output validation failed — (root): Invalid input" on nearly
every Bash command.
Track whether a sub-hook deliberately set stdout (string / {stdout}, e.g.
GateGuard) via a rawModified flag and emit '' in the pass-through case
instead of the echoed input. Preserves GateGuard pass-through and
block-no-verify's exit-2 blocking.
Update the three dispatcher tests that codified the buggy echo behavior to
expect empty stdout, and add a regression test for a plain pass-through
command.

fix: context-size /compact trigger, Codex marketplace plugin path, live README badges (#2237)
- suggest-compact hook now reads the latest usage record from the session transcript and suggests /compact at a window-scaled token threshold (160k/200k window, 250k/1M window; COMPACT_CONTEXT_THRESHOLD and COMPACT_CONTEXT_INTERVAL overridable), re-firing per 60k-token growth bucket; tool-call count stays as the secondary signal (#2155) - Codex repo marketplace now points at ./plugins/ecc instead of ./ — Codex never discovers plugins whose local marketplace source.path is the marketplace root (verified on Codex CLI 0.137.0); plugins/ecc is a thin folder referencing root skills/.mcp.json per maintainer direction on #2097; docs flag plugin mode as experimental with the upstream blocker openai/codex#26037 linked (#2128) - README badges for installs/stars/forks now use shields endpoint badges backed by api.ecc.tools (live install count 3,712 vs the stale static 150), which also eliminates shields' 'Unable to select next GitHub token from pool' render in the stars badge Closes #2155 Closes #2128 |

fix: stability batch — hook stdin truncation, Codex exa TOML, Stop hook JSON, GateGuard repetition (#2227)
* fix(hooks): fail open on oversized stdin instead of echoing truncated JSON (#2222) run-with-flags.js capped stdin at 1MB but every fallthrough path still echoed the truncated string to stdout. The harness parses hook stdout as JSON, got a document cut mid-stream, and blocked the tool call — so any Edit/Write with a >1MB hook payload was permanently blocked by every registered pre-write hook, before ECC_HOOK_PROFILE / ECC_DISABLED_HOOKS gating could run. - Exit 0 with empty stdout (no opinion) when the stdin cap trips, before any echo or gating logic. - Flush stdout via write callback before process.exit: exiting right after stdout.write() dropped everything past the ~64KB pipe buffer, cutting even sub-cap pass-through payloads mid-JSON. Regression tests cover the enabled, disabled, and missing-arg paths for oversized payloads plus full echo of sub-cap >64KB payloads. * fix(codex): stop emitting invalid exa url entry, align merge with connector policy (#2224) The Codex MCP merge declared exa with a url key, but Codex's [mcp_servers.*] TOML schema is stdio-only — the url key makes the entire config.toml fail to load, bricking both the codex CLI and the desktop app. Every install/update re-injected the line because the urlEntry branch treated the broken entry as present. - ECC_SERVERS now emits only the current default set per docs/MCP-CONNECTOR-POLICY.md: chrome-devtools (stdio, command/args). Retired servers (supabase, playwright, context7, exa, github, memory, sequential-thinking) are never re-emitted; existing user-managed entries are untouched. - The merge now repairs the exact ECC-emitted broken form (url-only exa entry) on every run so re-running the installer fixes broken configs instead of preserving them. User stdio exa entries (command + mcp-remote) are left alone. - check-codex-global-state.sh requires chrome-devtools instead of the retired set, and flags url-only exa entries with a repair hint. Tests cover repair, re-run idempotence, stdio-entry preservation, and no-retired-server emission in add, update, dry-run, and disabled modes. * fix(hooks): never echo truncated stdin from Stop hooks (#2090) Stop hooks follow the ECC pass-through convention (echo stdin on stdout), but every echoing Stop hook capped stdin and echoed the capped string. The Stop payload carries last_assistant_message, so a long final assistant message produced a JSON document cut mid-stream on stdout, which the harness reports as 'Stop hook error: JSON validation failed' across the whole Stop chain. Reproduced: a Stop payload with a >64KB last_assistant_message run through run-with-flags + cost-tracker emitted exactly 65536 bytes of invalid JSON (cost-tracker capped stdin at 64KB — far below realistic Stop payloads). - cost-tracker: raise the cap to 1MB (matching all other hooks) and suppress the pass-through echo when stdin was truncated. - check-console-log, stop-format-typecheck, desktop-notify: suppress the echo when stdin was truncated; flush stdout before process.exit so sub-cap payloads are not cut at the ~64KB pipe buffer. - All hooks keep exiting 0 (fail-open); diagnostics go to stderr. New stop-hooks-stdout test asserts the contract for every registered Stop hook: stdout is empty or valid JSON, exit code 0 — for realistic 100KB payloads and oversized >1MB payloads, via the production runner and via direct invocation. Updated the old hooks.test.js case that codified the truncated-echo behavior. * fix(hooks): dampen GateGuard fact-force repetition in long sessions (#2142) In long autonomous sessions the fact-force gate produced 10+ near-identical 'state facts -> blocked -> restate -> retry' blocks in one context window, which measurably raises the odds of the model collapsing into a degenerate single-token repetition loop. - Track a per-session fact_force_denials counter in GateGuard state (merged max across concurrent writers, reset with the session, robust to malformed on-disk values). - The first GATEGUARD_FACT_FORCE_FULL_DENIALS denials (default 3) keep the full four-fact block; later denials emit a condensed single-line message that carries the denial ordinal, so consecutive denials are structurally different and never textually identical. - True retries of the same target remain allowed without re-prompting (unchanged). Destructive-Bash and routine-Bash gates are unchanged, as are the ECC_GATEGUARD=off / ECC_DISABLED_HOOKS escape hatches. Eight new tests cover budget counting, condensed format, ordinal advancement, retry pass-through, env tuning, malformed state, MultiEdit dampening, and destructive-gate exemption. * fix(hooks): keep security hooks able to block on oversized stdin (#2222) Refine the truncation fail-open: instead of skipping the hook entirely, the runner now suppresses only its own raw-echo when stdin was truncated. The hook still executes and receives the truncated flag (run() context / ECC_HOOK_INPUT_TRUNCATED), so config-protection keeps blocking truncated protected-config payloads (its test requires exit 2) while pass-through hooks fail open with empty stdout as before. * style: apply repo formatter to touched hook files |

fix(hooks): stop false loop warnings and repeated identical context warnings (#2121)
* fix(hooks): stop false loop warnings and repeated identical context warnings
Two PostToolUse monitor defects surfaced during a long single-turn session:
1. ecc-metrics-bridge hashToolCall fingerprinted Edit/Write/MultiEdit on
file_path ONLY, so several distinct edits to the same file produced the
same hash and tripped the loop detector ("stuck loop") even though every
edit was different. Now the hash includes the edit content
(old_string/new_string/content/edits) so distinct edits to one file hash
differently; identical edits still collide as intended.
2. ecc-context-monitor re-emitted the SAME warning every DEBOUNCE_CALLS (5)
tool calls even when nothing changed. Because the cost figure only refreshes
at Stop (turn) boundaries, a single stale value printed the identical
warning ~20 times within one turn. Dedupe on message content instead: a
warning surfaces only when its text changes (cost moved, new file count, new
loop) or on first escalation to critical, and is otherwise suppressed.
Adds regression tests for the same-file/different-content hash case.
Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>
* fix(hooks): address CodeRabbit review (#2121)
- ecc-context-monitor: clear dedupe state when warnings resolve, so the same
warning text recurring in a later turn (context dips/recovers/dips, a loop
that stops then restarts) is surfaced again instead of suppressed as a
duplicate. Guarded so the no-warning hot path stays write-free.
- ecc-metrics-bridge: hash the FULL serialized edit payload and truncate the
digest, not the input. Slicing the serialized string to HASH_INPUT_LIMIT
first could collapse large edits sharing their first 2048 chars, reviving the
false-loop collision for big Write/edit payloads.
- Add regression test for >2048-char edit divergence.

fix(gateguard): gate force/path git checkout as destructive (#2158)
* fix(gateguard): gate force/path git checkout as destructive The destructive-command gate's `checkout` handler only flagged `git checkout -- <path>`. fix(dev-server-block): stop blocking dev-<suffix> scripts (#2179)
`DEV_PATTERN`'s trailing `\b` treats a hyphen as a word boundary, so `dev\b` matched the `dev` prefix of distinct npm scripts like `dev-setup` / `dev-docs` / `dev-build` and blocked them with exit 2. Replace the trailing `\b` with `(?![\w-])` so the dev server still matches (`dev`, `dev;`, `dev:ssr`) but `dev-<suffix>` scripts pass. fix(session-end): preserve $-sequences in user messages when rewriting summary (#2180)
The regenerated summary block embeds raw user-message text and was passed as the *replacement* argument to String.prototype.replace, where $-sequences ($&, $$, $`, $') are special. A user message containing $& re-injected the entire matched block (duplicating the summary markers) and $$ collapsed to $, silently corrupting the persisted session summary. buildSummarySection only escapes newlines and backticks, not $. Fix: use function replacers (() => summaryBlock) at both rewrite sites so the replacement text is treated literally. Adds an end-to-end regression test. fix(observer): auto-scale max_turns by analysis batch size (#2062)
* fix(observer): auto-scale max_turns by analysis batch size (#2035) The hardcoded default of MAX_TURNS=20 is insufficient when MAX_ANALYSIS_LINES=500 (also the default). Claude exhausts its turn budget before it can write all discovered instinct files, producing: Error: Reached max turns (20) Fix: when ECC_OBSERVER_MAX_TURNS is not explicitly set, compute max_turns proportionally to the actual analysis batch size: max_turns = clamp(analysis_count / 10, 20, 100) This gives: - 20–199 lines → 20 turns (existing floor, unchanged) - 500 lines → 50 turns (resolves the reported failure) - 1000 lines → 100 turns (cap) Explicitly setting ECC_OBSERVER_MAX_TURNS still overrides the auto-scaled value, preserving the existing escape hatch. * test(observer): update max_turns test for auto-scaling; document validation The max-turns budget test in tests/hooks/hooks.test.js still asserted the removed literal max_turns="${ECC_OBSERVER_MAX_TURNS:-20}", which would fail against the new auto-scaling logic. Assert the auto-scale formula and the 20/100 clamp bounds instead. Also add the explanatory comment CodeRabbit requested above the max_turns sanitization block, clarifying it guards the explicit ECC_OBSERVER_MAX_TURNS override path. fix(.cursor/hooks): route block-no-verify through local hook to fix message-body false positives (#2107) (#2177)
Cursor hooks still called `npx block-no-verify@1.1.2`, the broken external package whose matcher over-matches: it blocks legitimate `git commit` whenever `--no-verify` (or `no-verify`) appears anywhere in the command string, including inside the commit message body. The Claude Code surface already routes through the in-repo `scripts/hooks/block-no-verify.js`, which performs flag-position-aware tokenisation and passes 25 regression tests covering every false-positive case from #2107. Add a thin Cursor wrapper (`before-shell-execution-block-no-verify.js`) that reads Cursor stdin, transforms to the Claude Code `tool_input.command` shape, delegates to the local hook's exported `run()`, and forwards exit code and stderr. Update `.cursor/hooks.json` to call the wrapper instead of the npx package. New 14-case test file pins the false-positive cases from the issue plus the still-blocked real bypass attempts. fix(session-start): support ECC_SESSION_RETENTION_DAYS opt-out + document env var (#2151) (#2163)
* fix(session-start): support ECC_SESSION_RETENTION_DAYS opt-out + document env var The retention pass for *-session.tmp files (issue #2151) landed previously, but the env var that controls it was undocumented in the README and rejected falsy values (0, off, disabled), silently falling back to the 30-day default. Users who want to keep all sessions for forensic or research workflows had no way to opt out. This patch: - Extends getSessionRetentionDays() so 0|off|false|disabled|never|none disables pruning entirely (returns null sentinel; default behavior unchanged). - Updates the call site in main() to skip pruneExpiredSessions when retention is null and emits a clear "[SessionStart] Pruning disabled via ECC_SESSION_RETENTION_DAYS" log line so the operator can tell pruning is off. - Documents ECC_SESSION_RETENTION_DAYS in the README "Hook Runtime Controls" section alongside the other ECC_SESSION_* knobs. - Adds three regression tests in tests/hooks/hooks.test.js covering opt-out via 0, opt-out via off, and garbage-value fallback to default 30.
