Bortlesboat dcf27b8361 perf(proximity): bound memory for large line ranges ...

Count overlap from merged inclusive intervals instead of enumerating every
covered line. Preserve the overlap coefficient and whole-file behavior.

Cover equivalence, input immutability and billion-line spans under a bounded
child heap.
