fix(hooks): quote opt-in hook paths in setup docs

Unquoted ${CLAUDE_PROJECT_DIR} in SDD-CACHE and SIMPLIFY-IGNORE setup
examples breaks when the project path contains spaces, the same class
of failure fixed for SessionStart in #214.
