Eleven positive trigger prompts ranked their skill #2-#3 because the
descriptions lacked vocabulary users actually say (follow-up to #351):
- debugging-and-error-recovery: regression vocabulary (worked yesterday,
what broke)
- documentation-and-adrs: 'document an architecture decision', design
reasoning ('documentation' and 'document' don't stem together)
- doubt-driven-development: cross-examining assumptions, stress-testing
a plan for hidden failure modes, high-stakes changes
- git-workflow-and-versioning: splitting a messy working tree into
atomic commits
- incremental-implementation: thin/verifiable slices, next task from a
plan, feature flags
- shipping-and-launch: what needs to be in place before shipping
- spec-driven-development: PRD, objectives and scope
- test-driven-development: red-green-refactor
- using-agent-skills: deciding which workflow applies to the work
- source-driven-development: verify against the official docs
('docs'/'documentation' don't stem together)
Trigger rank-1 rate: 86% -> 100% (76/76), no new collision warnings.
CI floor raised from 80 to 95 per the ratchet policy; README baseline
updated and the stale #351 tracking sentence rephrased as guidance.
Every SKILL.md linked the shared checklists as `references/<file>.md`, a
path relative to the skill's own directory. Those files live in the repo
root `references/`, two levels up, so all 18 links across 11 skills
resolved to paths that do not exist -- in the repo and in every
plugin-install layout (~/.claude/plugins/cache/..., ~/.codex/...).
An agent following the guidance -- using-agent-skills pointing at the
Definition of Done, for example -- hit a file-not-found on every one.
Rewrite the 18 links to `../../references/<file>.md`, which resolves
correctly from `skills/<name>/`. Text-only; no structure or tone changed.
Verified: all 18 links resolve; hooks/session-start-test.sh passes
(required by CONTRIBUTING.md when touching using-agent-skills/SKILL.md);
validate-skills, validate-versions, validate-commands and
validate-artifact-paths all pass.

Folds in the safety and consistency findings surfaced by running
the skill against itself in two cycles (single-model adversarial
review + cross-model verification via Codex).
Cross-model escalation is now offered in every interactive cycle,
and the CLI invocation path uses a read-only sandbox with prompts
piped through stdin to neutralize shell-escaping and prompt-injection
risks (verified against Codex 0.128.0 and Gemini 0.41.2 locally).
Internal consistency fixes from the adversarial self-review: the
adversarial prompt explicitly overrides persona templates; the
persona roster is referenced rather than duplicated; the Step 2
'Claim' bullet is renamed Assertion to disambiguate from the Step 1
CLAIM block; the manual fallback in Loading Constraints is labeled
as a degraded last-resort path; the forward reference to Step 3 is
anchored; the Verification checklist no longer contains an item not
derivable from the Process; and using-agent-skills's Lifecycle
Sequence now includes the skill at position 7.
Codifies adversarial fresh-context review as an in-flight development
posture, distinct from /review (post-hoc PR-level verdict). Every
non-trivial decision passes through a 5-step cycle (CLAIM → EXTRACT →
DOUBT → RECONCILE → STOP) before standing.
Cross-model escalation is described as a contract, not as hardcoded
CLI invocations — the skill instructs the agent to ask the user which
external tool to use and how, since CLI syntax (Gemini, Codex, etc.)
changes and varies by environment.
