Document redirects, timeouts, page value redaction, HttpOnly cookie sources
and header refusals for request steps. Refusals shared by environment-build
and export_environment_bundle no longer name CLI flags, and the retained
loopback refusal no longer prints an empty file name. The skill reference
asks for read-backs after each write, shows the coverage view's source, names
package_tarball for unreleased builds and the browser path a scenario ships.
