The write-plan branch's page cap, observation and visibility fixes meet this branch's per-line rules: a page the collection has outgrown answers from its recording on every line that lists its whole collection, as the gate skips such reads on those lines; WriteEffects keeps both `removed` and `shows`. To stay under the line limit, item reads share one field check, reinsertion moves into SimulatedState, and `removed` takes the gone entity.
